This program is a 14 week condensed version of the Group Lifestyle Balance Program, led by certified leaders, for people who have been identified with, or are at risk for prediabetes and/or metabolic syndrome. This program is being offered by the Two Rivers Family Health Team. 

 

This program is offered in groups of 10-16 participats and focuses on helping individuals adopt and maintain lifestyle skills, including weight loss, nutrition and physical activity. 

  

The program is FREE to attend and is funded by the Ontario Ministry of Health and Long Term Care (MOHLTC).  It is based on the University of Pittsburgh Diabetes Prevention Support Centers’ program.
