Is your patient ready to attend a chronic disease management program?

 

 

 

 

It is important to assess readiness to change before booking a workshop, otherwise your patient is not likely to attend. 

 

 

Perform the "stop-light" assessment as follows.  Unless a person says YES to all 4 of the following questions, don't expect change to happen easily. 

 

  1. Do you view X as a problem?

  2. Does X concern you or cause distress?

  3. Are you interested in changing?

  4. Are you ready to change now?

 

If your patient answers yes to only 1 of the answers, STOP and reassess at next appointment.

If your patient answers yes to 2 to 3 of the answers, PROCEED WITH CAUTION and provide them with a prescription to attend a class and recommend they call or go online to book a class.

If your patient answers yes to all 4 of the answers, GO.  They are ready to commit to a class.  Click here to book a workshop.
