Our self‑management program offers free, supportive workshops to help you and your loved ones build confidence, skills, and community so you can live well and take charge of your health.

  • Adults (18 +) living with one or more chronic or long-term health conditions

  • People at risk of developing a chronic condition

  • Caregivers, family members, and friends who support someone living with a chronic illness

  • Loved ones seeking tools, understanding, and support for navigating the journey alongside someone else

Why

Self‑Management Matters

 

Medical care (doctors, specialists, medications) is critical — but living with a chronic condition happens every day, often without direct care. Self‑management helps you:

 

  • Understand your condition and how it affects your body, mind, and life

  • Develop skills to manage symptoms, stress, sleep, mood, and daily challenges

  • Set personal goals that matter to you and take small, manageable steps

  • Connect with others who understand — building community, reducing isolation

  • Thrive despite health challenges — with support, education, and shared experience

It’s easy to get involved. You can self-refer — no doctor referral is needed. Workshops fill up quickly since spaces are limited, so we encourage early registration. Sessions are free. Whether you join online or in person, you’ll receive workshop materials, a supportive group environment, and trained facilitators guiding the way. Looking for a topic that's not on our list?
